On March 1, 1985, the Northeast Recidivism Team (NERT) administered by San Francisco Medical Center Outpatient Improvement Programs, and under the direction of the SFGH Department of Psychiatry, formally became Citywide Case Management, a UCSF/SFGH Department of Psychiatry outpatient program. Recognizing that exceptional inpatient treatment meant little if high risk clients received poor follow-up care in the community, the program grew from serving high risk clients in the city's northeast sector to stationing case managers in seven outpatient clinics throughout San Francisco, focusing on treating high users of inpatient services.
Today known as the Division of Citywide Case Management Programs, it supports the recovery of San Francisco’s highest risk mentally ill adults with the goal of reducing their use of institutional and acute care (psychiatric emergency services, hospital care, and/or jails), while helping to maximize their ability to maintain stable, productive, fulfilling lives in the community. All of the division’s program services include medication assessment and management, crisis intervention, outreach and case management, and individual, group and family therapy, which continue as long as clients need an intensive level of intervention.
